Knowledge and Attitude on Challenges towards Nursing Education and Clinical Practices

ABSTRACT:

Nursing education involves provision of practical training with the purpose to prepare students for future duties as nursing care professionals. Clinical learning is a basic factor of nursing education. The clinical learning environment is essential for a student to most likely to accomplish desired learning outcomes. The study aimed to assess the knowledge and attitude on challenges towards education and clinical practices among nursing students. This study used a qualitative research approach. The study was conducted at C.S.I Eliza Caldwell College of Nursing. The population consisted of 100 students doing Bachelor of Nursing degree program were selected by non-probability convenient sampling method. After obtaining written consent from the nursing students, data collection was commenced. A brief explanation on the study was given to the participants. Data pertaining to the demographic variables were collected by interview method. Resource material such as a valid and reliable self-structured questionnaire and 5-point self-structured Likert scale was used. Data were collected for 2 weeks. Each sample took 20-30 minutes to answer the questions. Data was analyzed using descriptive and inferential statistics. The paired t‟ value is 4.213 and the p-value is 0.000, the level of significance (i.e., 4.213) Therefore, the H1 hypothesis shows there is a significant relationship between the knowledge and attitude among nursing students. There is a positive correlation (0.420) between knowledge and attitude on challenges towards education and clinical practices. Thus, the study concluded that increasing knowledge and attitude will lead to face the challenges towards education and clinical practices among the nursing college students.

INTRODUCTION:

Nursing is a hands-on profession that requires a meaningful combination of theory with practical skill, and these two complements each other (UzelliYılmaz and AkınKorhan, 2017).

 

Nursing as a practice-based profession, requires clinical learning environment to equip nurse students with professional abilities by combining cognitive, psychomotor, affective skills and coaching the students. A large part of nursing education is carried out in clinical environments. Therefore, clinical education is an essential and integral part of the nursing education program.

Nursing students, exposure to clinical environment is the most important element affects teaching process in clinical settings. Reorganization difficulties of nursing students in the clinical learning environment could improve quality of skills and performance of nursing students (Jamshidi, Molazem, Sharif,Torabizadeh, and Najafi Kalyani, 2016). Nursing students conduct their clinical practice in various kinds of clinical settings. The clinical learning environment is essential for students to accomplish desired learning outcomes most likely.

 

Gap between theoretical learning and its implementation during training has impact on nursing student’s empowerment. Nursing students have emphasized education which taught by preceptor during their training but afterword in their practice clinical experience has been totally difference. In the literature, it is stated that the reasons for challenges experienced by nursing students include a lack of clinical practice areas, a shortage of professional nurses and instructors, high number of nursing students, negative approaches of healthcare personnel, communication problems, insufficient laboratory, and classroom environments (Drateru, 2019).

 

By unyielding so many reviews of literature it was found that studies regarding knowledge and attitude on challenges towards nursing education and clinical practices are still lacking. Further research is required to strengthen the evidence. So, this study aimed to assess the knowledge and attitude on challenges towards nursing education and clinical practices among nursing students. Researchers have studied the opinions of nursing students when they approach their challenges towards academic and clinical training. The results of this study provide important information in solving the difficulties of nursing students, which may hinder their academic as well as clinical performance. The success of nursing students is particularly important that they must provide quality care to patients, family members and the community.

 

NEED AND SIGNIFICANCE:

Nursing is considered a noble profession and the future of nursing is dependent upon the student nurses who will lead the nursing in the future. Nursing is a practice-based profession that is built on theoretical knowledge, both of which gained through nursing education program. Practice is especially important with nursing students because it helps them move from a theoretical study to practice nursing skills and familiarize themselves with the hospital environment. Nursing students also can practice and work as nurses in medical facilities. This helps the students understand how their future work will be and is essential for the students to adapt to the nursing field.

 

Recent studies have shown that, nursing students face many difficulties that affect their learning skills and academic performance. By identifying and recognizing the difficulties encountered by students during their years of study, measures can be taken to reduce barriers and improve academic performance (Fooladi et al. 2022). Mthimunye and Daniels (2020) concluded that nursing students face difficulties such as off-campus stay, curriculum structure, insufficient academic and clinical support, and unfavourable physical faculty environment.

 

According to GünayandKılınç et.al, 2018 the aim of the clinical training is to enable students to transfer theoretical education into practice, to identify and solve problems, to develop their skills, and facilitate the development of competencies. The purpose is to train nurses who can adapt to and be successful in the ever-changing practice environment

 

In India, statistics report on attitude of nursing students towards nursing profession shows 95.97% of participants found positive attitude and only 4.02% found negative attitude towards nursing profession (Sushil Humane 2022). The  findings  of previous research  indicated  that  the most important challenge in the educational  environment were educator follow traditional ways of teaching in their nursing care (97.1%),no application of theoretical aspects of the nursing process in practice  (69.6%), there is a gap between practice and education system (98.1%), the use of practice were follow as there is a major difference what they were taught in class is similar to what they practice at clinical environment  (95.8%), student not received constructive feedback from nurse educator/clinical facilitator during accompaniment (96.2%).Study conducted from the students of  Iranian Medical University shows  that imperative theoretical and clinical issues(88.9%) related  to academic are not practiced in clinical areas,(85.6%) theoretical aspects of the nursing procedure are not being  applied in practice (81.1%) teachers are not knowledgeable and professional educators.(75.6%) are not follow traditional routine-oriented ways in their nursing care.

 

This study will give enough information regarding knowledge and attitude on challenges towards nursing education which influences nursing student’s professional growth. On behalf of this study, researcher will minimize the problems in theoretical and clinical practices among nursing students through seminar and workshops, which enhance the theoretical and clinical performance of students. Furthermore, quality of learning will be improved in the institution which will enhance institutional prestige. The findings will help the students as well as instructors to minimize their problem in the improvement of clinical skills, competences, and academic performances of the students.

STATEMENT OF THE PROBLEM:

A descriptive study to assess the knowledge and attitude on challenges towards education and clinical practices among nursing students at selected Nursing College, Tirunelveli District, Tamil Nadu.

 

OBJECTIVES OF THE STUDY:

1.     To assess the knowledge and attitude on challenges towards education and clinical practices among nursing students.

2.     To correlate the knowledge and attitude on challenges towards education and clinical practices among nursing students.

3.     To find the association between the knowledge and attitude on challenges towards education and clinical practices among nursing students with selected demographic variables.

 

REVIEW OF LITERATURE:

Silvana Gripshi et.al (2022) performed a qualitative research method on Challenges of Nursing Students in Relation to Academic Performance at the Faculty of Technical Medical Sciences. Data were collected and analysed through thematic analysis among 51 Bachelor students. Semi-structured discussions were held. The study findings found that nursing students face difficulties such as low level of students’ engagement in clinical practice, passive teaching methods, unfair and non-transparent evaluation, large number of exams per season, insufficient time, employment and finances, physical environment in the faculty and public transport.

 

Shahram Baraz, Robabeh Memarian (2018) performed a qualitative study in Iran on Learning challenges of nursing students in clinical environments.18 undergraduate nursing students were selected by purposive sampling method from Shahid Beheshti Universities Tehran. Semi-structured interviews were used to collect data. The results show from the data analysis, which represented the student’s clinical learning challenges included insufficient qualification of nursing instructors and unsupportive learning environment. Thus, the study concluded that identification of the students learning challenges and actions to remove or modify them will create more learning opportunities for the students, improve the achievement of educational goals, provide training to nursing students with the needed competencies to meet the complex demands of caring and for application of theories in practice, and improve the quality of health care services.

 

METHODOLOGY:

This study used a qualitative research approach. The study was conducted at C.S.I Eliza Caldwell College of Nursing. The population consisted of 100 students doing Bachelor Nursing degree program were selected by non-probability convenient sampling method. Participants were informed that the principle of confidentiality and anonymity would be respected.  Self-introduction about the investigator was done. The investigator introduced the purpose and methods of research and asked permission to investigate the nursing students. After obtaining written consent from the nursing students, data collection was commenced. A brief explanation on the study was given to the participants. Data pertaining to the demographic variables were collected by interview method. Resource material such as a valid and reliable self-structured questionnaire and 5-point self-structured Likert scale was used. Data were collected for 2 weeks. Each sample took 20-30 minutes to answer the questions. Data was analyzed using descriptive and inferential statistics.

 

FINDINGS:

As the calculated paired t‟ value is 4.213 and the corresponding p-value is 0.000, the level of significance (i.e.,4.213) is less than 0.05, the null hypothesis is accepted at 5 per cent level of significance. Therefore, the H1 hypothesis shows there is a significant relationship between the knowledge and attitude among nursing students.

 

There is a positive correlation (0.420) between knowledge and attitude on challenges towards education and clinical practices.

To associate knowledge, attitude with selected demographic variables towards education and clinical practices:

The third objective is to associate knowledge, attitude and demographic variables which shows that age, education, fathers’ occupation, income training year, previous experience in hostel, religion, residence and challenges in clinical practices is not significant at 5% level of significance. Therefore, the hypothesis is rejected. It means that all the demographic variables are influencing the knowledge and attitude of the nursing students.  Thus, the study concluded that increasing knowledge and attitude will lead to face the challenges towards education and clinical practices among the nursing college students.

 

RECOMMENDATIONS:

Based on the study findings the recommendations are:

1.     Similar study can be replicated on a larger sample to increase validity and generalizability of findings.

2.     An explorative study can be done at various settings to identify factors influencing challenges towards nursing education and clinical practices.

3.     A comparative study can be done to ascertain knowledge and attitude of students regarding nursing education.

4.     Similar study can be replicated using structured teaching programme regarding nursing education and clinical practices.

 

CONCLUSION:

On the basis of present study findings, the researcher found that 37.93% of nursing students were having inadequate knowledge and 33.07% of nursing students have unfavourable attitude towards higher education, so more focus should be given to increase the quality care to patients and standards of our profession. The present study was informative and enriching for the investigator. The respondents were very cooperative for the study. This study was a new learning experience for the investigator. The constant directions, encouragement, suggestions, and valuable corrections contributed their maximum for the fruitful completion of the study.
